At FunctionSmart, we accept most PPO plans and some HMO plans that have contracted with us. We will work with you to determine insurance coverage prior to your first visit so you will know what your coverage is.

We are contracted with the medical groups listed to the right, with some only for pediatric pelvic floor therapy.
For ones NOT listed, we are out of network as we do not have a direct contract with them. Unfortunately, we are unable to accept any new single case agreements at this time.

- CPMG/RCHN (including Molina, CHG, and Blue Shield Promise) for pediatric pelvic floor and orthopedics
- HealthNet HMOs through Direct Network and some medical groups (OON with UCSD Medical Group)

Affordable Self Pay Options
For anyone who is out of network or seeking services that are not covered by the terms of their plan, we do have self pay options and will provide current rates on request prior to scheduling.
We will also provide a “Good Faith Estimate” for the evaluation if requested and for any follow up care once the evaluation has been completed.
